Benefit Details

1. To take advantage of our preferred pricing, you must contact a Care Manager directly (Toll-Free telephone numbers and website address will be provided upon registration.)
2. Our Care Managers provide a thorough prescreening process along with important information about LASIK techonogies and procedures.
3. LASIK Care Managers schedule an appointment with the selected provider and collects a fully refundable deposit from the member.  This deposit is 100% refundable for any reason as long as the procedure was not performed.
4. If you choose to finance, your monthly payments can be as low as $42 per month for both eyes (9.9% APR for 60 months.)
